HOSPICE CARE

Hospice care is for anyone who is terminally ill –- not just patients with cancer. By bringing the hospital to the patient, hospice provides holistic and personalized care that is focused on comfort, rather than a cure. Care is provided by an interdisciplinary team of professionals who consider the whole person, including the individual’s physical, emotional, social and spiritual needs. Hospice care is available 24 hours a day, seven days a week, in the comfort of your home or community.

Hospice care is unique because it provides care beyond the patient, embracing caregivers and family. This holistic picture offers peace of mind and promotes a better quality of life for everyone involved.

You or your loved one may qualify for hospice sooner than you think, and we believe it is important to explore the option early. With holistic, personalized care, patients not only enjoy a better quality of life, but a study sponsored by the National Hospice and Palliative Care Organization has shown that patients can actually live longer with hospice.

Dignity Team Health is known for its hospice, palliative and related care. We provide exceptional levels of comfort, compassion and support to patients with life-limiting, progressive illnesses, as well as their caregivers and families. We are Texans, just like you, and you can trust that we will always be real, authentic partners with you throughout this difficult journey.

Our mission is to help our patients lead lives of quality and dignity. We know that families struggling with issues during and after the final stages of life often mourn uncomforted. That’s why we continue to care for these families for 13 months after a patient passes, so they know they are not alone.

“To have a company that will make decisions in the best interest of the patient and to make a decision immediately is a huge factor in our recommendation of Dignity and all of their staff.”

Hospice services are designed for people in the last six months of their life. Instead of spending their final days in a hospital or clinical setting, hospice allows for a patient to continue to live at home, surrounded by friends, family, and familiar surroundings. Caregivers come to your loved one, not the other way around.

The number one goal of hospice is to make a patient’s remaining time on this earth as comfortable and stress-free as possible. At the same time, hospice also offers emotional support to the patient’s families. Hospice services are fully covered by Medicaid and Medicare, making them affordable and accessible to families everywhere.

  • Routine care for the patient. This covers a wide variety of duties, all of which see to the patient’s physical, emotional, and spiritual well-being. Pain and symptom management (including the administration of medication) and nutrition management (including feeding) are critical aspects. Dignity can also provide the equipment necessary for a patient’s care, whether it’s as simple as incontinent supplies and oxygen or as complex as hospital beds and wheelchairs.
  • Continuous Care. In addition to routine care, our nursing staff will keep an eye out for—and, if necessary, resolve—any opportunistic infections or acute medical conditions that arise unexpectedly.
  • Respite care for primary caregivers. To help combat caregiver burnout, we offer respite care for our patient’s loved ones. This allows you to “take a break” for up to five days while our staff watches over the patient in one of our affiliated long-term care facilities.
  • General In-Patient Care. Even if your loved one’s condition makes it impossible or impractical for them to continue living at home, we will still oversee the care they receive in a hospital or medical facility setting.

Our staff is made up of an extensive team of nurses, care specialists, social workers, chaplains, nutritionists, pharmacists, bereavement counselors and general volunteers. They work together with one goal in mind: To make sure that their patients receive the best end-of-life care possible. And because all Dignity Hospice employees and volunteers undergo criminal background checks and drug testing, you can rest assured that your loved one is in safe, responsible hands. c
